Ballarat Health Services (Social Work Department)

End of Treatment Care Planning

This is a 6 month pilot project to be run in the Radiotherapy Unit (BAROC) at the Ballarat Regional Integrated Cancer Centre and led by the Social Work department. It will allow patients to participate in Care Planning towards the end of treatment to assist with readjustment to life after cancer. Patients will develop their own goals with the support of the treating team and be provided with follow up review and support 3 months after treatment end. 

Care Plans may address one or more of the following: 

  • Questions about likely course of recovery
  • Follow up visits
  • Long term side effects of treatments
  • Personal goals / priorities
  • Financial concerns
  • Emotional issues
  • Lifestyle changes
  • Readjustment
  • Sexuality issues
  • Relationship and carer concerns

Project progress
  • A steering committee has been established and meets monthly.
  • A validated assessment tool has been chosen and approved.
  • Ethics approval is required from both the Austin Health Human Research Ethics Committee (Austin being the primary site) and the BHS & SJOG Human Research Ethics Committee. 
  • The application to the Austin Health Human Research Ethics Committee has been prepared and submitted for the Feb 1st meeting. Once approved by the Austin ethics approval will be forwarded to BHS ethics for endorsement.
  • The pilot will start when all ethics approvals have been provided
