GICS has formed strong relationships with the people in the health services who deliver cancer care across the Grampians region.  We are grateful to those who have invested time and effort to work with us and are proud of the achievements we have made together.

The aim of this website is to provide helpful resources to those who work with people affected by cancer.  If you have suggestions for ways to improve this website to better meet your needs, please complete the Feedback Form and we will do our best to accommodate your request.

Multidisciplinary Meetings - Patient Request Form:

Click here to download, print and complete the Patient request form and fax to the GICS office on 5320-4076 at least one week prior to the required meeting date.  (Ensure that you indicate the meeting type and date on the top of the form)

Familial Cancer Clinics - Service provided by Austin Health Genetics Service (9496-3027 for appointments)

General Genetics Clinics - Service provided by Genetic Health Services Victoria (8341-6297 for appointments for both paediatric and adult patients)
