MDM Patient Informed Consent formsA template form is available from the GICS office which can be adapted to your setting. The purpose of this kit is to formalize the patient consent process and to be a record of patient consent. The kit contains a letter to the patient explaining the MDM and why the clinician wants the patient to be discussed, and also a signature page for the patient to sign.
