The Patient Request form can be downloaded then printed, completed by the requesting clinician (please fill in which meeting type and date) and then faxed back to GICS office on (03) 5320-4076.  Please include as much detail as possible.

MDM Patient Informed Consent forms

A template form is available from the GICS office which can be adapted to your setting.  The purpose of this kit is to formalize the patient consent process and to be a record of patient consent.

The kit contains a letter to the patient explaining the MDM and why the clinician wants the patient to be discussed, and also a signature page for the patient to sign.

Clinical Guidelines for the Administration of Oral Chemotherapy Agents in the Community Setting

The need for improved service delivery along with improved outcomes for
patients and their families has been identified as a priority for both State and
Federal governments.
Victoria has developed a significant cancer reform agenda that seeks to improve
the planning and delivery of treatment and support to patients’ so that the
appropriate care is provided in a timely manner as close to the patients home as
possible.
