MDM Patient Informed Consent forms A template form is available from the GICS office which can be adapted to your setting. The purpose of this kit is to formalize the patient consent process and to be a record of patient consent. The kit contains a letter to the patient explaining the MDM and why the clinician wants the patient to be discussed, and also a signature page for the patient to sign.
